The timelines below outline the recommended post-secondary planning steps students should complete during each year of their high school career. Service Learning Verification Form

After each service learning experience, students must print the Service Learning Verification Form (double-sided), which is also available for pickup in the High School Guidance Office. The student is responsible for completing the reflection/journaling section on the back of the form and having the front of the form signed and completed appropriately. Once fully completed, the form should be submitted to the Guidance Office for review and documentation.

Visitor Shadowing Form

The Student Visitor’s Form is used to request approval for a Garden Spot High School student to bring a visiting student to school for a specific day. The form collects basic information about both the host student and the visitor, including their names, grades, school attended, and confirmation that the visitor’s immunizations are up to date. It also requires a parent or guardian signature and signatures from each of the host student’s teachers to confirm permission for the visitor to attend classes. A brief reason for the visit must be provided, and final approval is granted by school administration.
