This form is intended for Garden Spot High School alumni or former students who attended GSHS but did not graduate and wish to request an official transcript.

To request a transcript, please complete the form and return it to Tanya Laudermilch. Requests should be submitted at least 10 business days before any application or employer deadline.

Completed forms can be submitted by mail and email.

There is no fee for transcript requests.

This form is for current Garden Spot High School students only. If you are a graduate or former student, please use the Alumni Transcript Release Form instead.

A final transcript will be sent to the college indicated on your end-of-year senior survey.
